What it is

Health Assessments at Flourish

A health assessment is a longer, structured appointment that looks at the whole picture rather than one problem. Medicare funds several of them and at Flourish they are bulk billed.

  • Health Assessment for people aged 75 and over
  • Aboriginal and Torres Strait Islander health check (from any age)
  • Health assessment for people with an intellectual disability
  • Refugee and humanitarian entrant health assessment
  • 45–49 year old health check for those at risk of chronic disease
  • Type 2 diabetes risk evaluation for people aged 40–49 with a high score on the AUSDRISK questionnaire
  • Review of medications, memory, mobility, hearing and vision
  • Referral to allied health and community services

What happens

What to expect

01

A long appointment

These take time by design. Our practice nurse may complete part of the assessment with you first.

02

A full picture

Medications, past history, how you are managing at home and anything that has changed.

03

With your GP

Your GP reviews the assessment with you and discusses anything that needs attention.

04

Follow-up

You may leave with referrals, a care plan or simply a clear all — and a date for the next one.

Common questions

Common questions

Most Medicare health assessments are available once every twelve months. Your GP will tell you when you are next eligible.

No. Book directly with your GP.

All your current medications, including anything from the chemist or supermarket, plus glasses and hearing aids if you use them.

No. Health assessments are bulk billed for eligible patients with a valid Medicare card.

If you are aged 40 to 49, a short questionnaire called AUSDRISK estimates your risk of developing type 2 diabetes over the next five years. If your score is high, Medicare funds an assessment with your GP to look at the factors behind it — such as weight, physical activity, blood pressure and family history — and agree what to do about them. It is available once every three years. You can complete AUSDRISK with us at the appointment.
