New patients
Everything you need for your first appointment with us.
Getting started
02
A first visit takes longer than a routine one. Ask for a long appointment so your GP has time to take your history properly.
03
Medicare card, a list of your current medications and any letters or results from previous doctors.
04
There is a short new-patient form to complete before you go in.
On the day
This is what makes your appointment bulk billed. Bring any concession or DVA card too.
A written list or the boxes themselves. Include anything from the chemist or supermarket and any vitamins or supplements.
Specialist letters, discharge summaries, recent results and your immunisation record if you have one.
Transferring
You do not have to start from scratch. With your written consent, we can request a copy of your health summary from your previous GP.
Ask reception for a records transfer form or mention it at your first appointment. Your previous practice may charge an administrative fee for transferring records — that is their fee, not ours.
Your records belong to you. Changing GP does not mean losing your history.
