Who it is for and what it costs

Who it is for

Patients living with an ongoing condition — or several — that needs regular review. If you have been managing alone or your care has been spread across different clinics, this is a good place to bring it back together.

What happens

What to expect

01

A longer first appointment

Preparing a management plan takes time. Book a long appointment so nothing gets rushed.

02

Your plan

Your GP writes down your goals, your medications and who else is involved in your care.

03

Allied health

Where you are eligible, a Team Care Arrangement can give you Medicare-subsidised visits to allied health providers.

04

Review

We schedule reviews and remind you when they are due, so your plan stays current.

Common questions

Common questions
